VASER Liposuction

Latest, third generation ULTRASONIC liposuction technique that is designed to virtually eliminate bleeding during surgery and to reduce inflammation and bruising. VASER Ultrasound technology literally liquefies the fat, which allows for greater precision and vastly superior results.
Ideal candidate for Vaser Liposuction
All candidates for liposuction should have realistic expectations of the results that can be achieved through the procedure and should be in good physical and mental health. The best candidate is someone who is in excellent health, is within 25 pounds of his or her ideal body weight, and has deposits of fat that have not responded to diet or exercise.   
At the consultation
You should express your desires and expectations from the surgical procedures. The surgeon will evaluate your medical health and explain the surgical techniques available to you. Your question will be answered as thoroughly as possible.  Examinations of your trouble are will be performed. The surgeon will go over the risks and potential complications of the procedure, as well as, the pre-operative and post –operative process.


Risk Factors
All surgeries carry some risks. Listed below are the risks or potential complications:
The risks of VASER liposuction are very similar to the liposuction risks associated with any other method. However, because LipoSelection minimizes damage to other body tissues, patients typically experience less pain and fewer side effects than with other forms of liposuction. Serious liposuction complications are quite rare. Fat clots or blood clots may form, and there is a chance these will migrate to the lungs and cause serious illness. The fluid loss that results from liposuction will be anticipated by a qualified doctor, as it can cause low blood pressure, friction burns, or fluid buildup. The LipoSelection probe’s ultrasound energy may injure the skin or deeper tissues; although in most cases the benefits of the ultrasound system make it safer than traditional liposuction. If a doctor inexperienced with the LipoSelection procedure applies too much fluid, the wetting solution may collect in the lungs. If there is too much lidocaine in the liposuction solution, lidocaine toxicity may develop.
 
Before the surgery
At this time, you will be given an opportunity to ask any further questions. Continuation of healthy diet and exercise is very important before the surgery. Smoking must be stopped at least two weeks before the surgery
 
On the day of the operation
No eating or drinking for 8 hours before the surgery.  You should wear loose fitting clothing and sneakers. Do not wear sandals or slippers. You will meet with our nurse to prepare you for surgery. The surgeon will discuss the procedure with you again and do pre-operative planning. You will meet with our anesthesia provider to discuss anesthesia. You will be given a choice of day surgery or general anesthesia which required at least one day hospital stay depend on your choice but normally if it is an extensive surgery,   we required 4 days and 3 night’s hospital stay.
 
Anesthesia used
This procedure can be performed using local or general anesthesia.
 
 During the surgery
First, the area to be sculpted is filled with a special saline solution that helps numb the site and shrink the blood vessels (to minimize blood loss and reduce bruising). This solution wets and fills the area to be treated, making fat easier to break up with the VASER System’s sound energy. Then a small probe transmits sound energy (similar to that used for cataract removal in the eye) that breaks up fat on contact, while leaving other important tissues relatively intact. Finally, gentle suction of the sculpted site removes the emulsified fat quickly. Thanks to its patented technology, only the LipoSelection procedure first emulsifies and then removes fat in this manner.
 
Post Operative Expectation
The treated area will be wrap with compression garments snugly and continuously for 6 days-2wks. Usually drains are placed removing excess fluid over a period of 2-3 days.
 
 Back to normal work
Regular exercise work out may start after 2 weeks, start with light exercise then gradually move your way up to your regular routine as tolerated by your body.
 
Pre-Operative instruction
  1. Consultation with surgeon
  2. Evaluation for other medical conditions
  3. ECG and Blood test will be taken
  4. No intake of food or water 6 hrs prior to surgery
  5. Compression garments should be available to be used post-op
  6. Stop smoking and alcohol intake at least 2 weeks before surgery
  7. Anticoagulants, vitamins, hormones (including birth control pills), anti-depressants and some other medications also to be stopped at least 2 weeks before surgery.
     
Post-Operative instruction
  1. Wrap treated area with compression garments snugly and continuously for 6 days-2 weeks.
  2. A day after the surgery, patient may have light to moderate physical activities
  3. Some incisions will be left open and drainage is expected, absorbent pads should be used and changed frequently as needed.  
  4. Incisions will close by themselves within 2-3 days.  Use band aid to protect incision sites.
  5. Keep incision clean
  6. May have quick bath after 2 days post op. Regular shower when incisions are closed.
  7. May use regular girdle 2 weeks after compression garments.
  8. Do not apply ice packs or a heating pad to skin overlying the areas treated by liposuction
  9. Do not soak in a bath, Jacuzzi, pool or beach at last 7 days post-op
  10. Bruising may diminish after 2-3 weeks
  11. Return for check-up after 4 days removal of stitches.
  12. Swelling is expected, Maximal in the first 2 weeks, then gradually subsides.  Final result in 3-4 months.
  13. Skin may feel numb on the surface but insides are tender to touch.
  14. Some areas may become hard and lumpy; this is normal and will go away when fully recovered

Metoidioplasty, sometimes referred to as a meto or meta or spelled "metaoidioplasty", is an alternative to phalloplasty for trans men. With the effects of testosterone treatment, the clitoris enlarges, over time, to an average of 4-5 cm (due to the clitoris and penis being developmentally homologous organs).
In a metoidioplasty the enlarged clitoral tissue is released from its position and moved forward to more closely approximate the position of a penis.
Metoidioplasty is technically simpler than a phalloplasty, and has fewer complications. The surgery itself is also considerably shorter (2-3 hours vs. 8-10 hours overall) and it is much less expensive. Unlike a phalloplasty, an erectile prosthesis is not needed to achieve erection, although the erection is usually less hard compared to the penis of a cisgender man. Because metoidioplasty can not achieve penile dimensions comparable to a cisgender penis, the neopenis is usually not capable of penetrative sexual intercourse. Genital orgasm is almost always retained after surgery (unlike phalloplasty which carries significant risk of loss of erogenous sensation).

Non-Incisional Suturing technique is a minimally-invasive method that employs two sutures on each upper eyelid that are tied in three places and twisted around each other four or five times at the central tie. The threads can be tightened and the drooping upper lid can be elevated to a better position in more secure and therefore longer lasting results with no dissection of eyelid tissues is required.

Duration of the Operation:
The procedure can be done quickly, usually 15-30 minutes.

Post operative care:
You should rest preferably in bed on two pillows or in a recliner.  A bandage over the eyes is not needed. In the evening, you may take a bath. Avoid straining or bending over.  Watching television is acceptable. Apply clean (not sterile) cold compresses to the eyelids to prevent swelling. Pain after surgery is usually mild. You will be given pain medication.

Recovery Period:
It is normal for the eyelids to become red, swollen, and bruised for a few days and will gradually subside.  A small amount of bloody fluid draining from the wound is normal for a few days.  If you were given an antibiotic drop or ointment, gently apply a small amount to any stitches before going to bed, and then twice a day thereafter.

Labiaplasty

Labiaplasty is a surgical procedure that will reduce and/or reshape the labia minora - the skin that covers the female clitoris and vaginal opening.
During surgery, larger or uneven inner vaginal lips are cut and shortened using specialized scissors, a scalpel or a laser. In addition, the labia majora, on the outside of the body, can be reduced with either surgery or liposuction if needed. A hoodectomy, which exposes the clitoris in an attempt to increase sexual stimulation, may be performed at the same time.

Duration of the operation:

Gluteal Augmentation is a cosmetic surgical procedure that uses buttock implants to increase the size and reshape the appearance of the buttocks. Incision may be made in any one of the following places: where the cheek meets the back of the thigh, or down the buttock crease. The incision is usually made in the buttock crease, where scars are not noticeable. The surgeon then creates a pocket large enough to insert the butt implants. Non-Incisional Suturing Technique

More than thirty techniques have been described that involve placement of one or more permanent sutures into the eyelid from small incisions made on the back and/or front surfaces of the eyelid (while sometimes called "non-incisional," the designation is incorrect).

Preparation before Surgery
·        Get lab testing or a medical evaluation
·        Take certain medications or adjust your current medications
·        Stop smoking well in advance of surgery
·        Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding

Surgical Procedures
One variation known as DST (double sutures with twisting, double stranded twist, or durable suture technique, all of which indicate the same operation) employs two sutures that are tied in three places and twisted around each other four or five times at the central tie. While promoted as yielding a more stable or even permanent result, we have seen many patients who have undergone this operation elsewhere only to have their creases fade as early as one to eight months following surgery.

Post-Operative Care
You should rest as much as possible, preferably in bed on two pillows or in a recliner.  A bandage over the eyes is not needed.  In the evening, you may take a bath if someone is there to help you.  Avoid straining or bending over.  Watching television is acceptable.
 To prevent swelling, apply clean (not sterile) cold compresses to the eyelids as much as possible until you go to sleep.  Gauze pads or a clean towel may be soaked in a basin of ice cubes and applied directly.  Alternatively, a large Ziploc bag may be partially filled with ice cubes or frozen baby peas and placed over a moist gauze or towel.
Pain after surgery is usually mild.  Use Tylenol or any stronger pain medicine you were given. Avoid using aspirin or ibuprofen for two days.  Severe pain should be reported.
It is normal for the eyelids to become red, swollen, and bruised.  A small amount of bloody fluid draining from the wound is normal for a few days.  Occasionally, the eye itself may turn red and swell.  Brisk bleeding not responsive to pressure should be reported.
The eyelid creases will always look too high, too deep, overly rounded, and asymmetrical until swelling resolves and the deep tissues have a chance to soften. The eyelashes may appear to be pointed unnaturally upward.
If you were given an antibiotic drop or ointment, gently apply a small amount to any stitches before going to bed, and then twice a day thereafter.  If your eyes feel irritated, the antibiotic may be applied directly to the eye surface. Avoid the use of dry Q-tips on any stitches.

Possible Risks and Complication
Surgery may be complicated by irritation from sutures rubbing against the cornea of the eye. Since the lid is entered through multiple small cuts without full visualization of the internal eyelid anatomy, suture placement and surgical manipulation may cause tissue injury as well as uneven skin scars. Crease loss is common. If a patient who has undergone suture treatment later elects to undergo an incisional techn
ique, internal scarring and retained suture fragments may limit the precision of the second surgery.

Preparation before Surgery
During the consultation, you and your surgeon will discuss the changes that you would like to make in your appearance. He/she will explain the different options available to you, the procedure itself, and its risks and limitations. He/she will also explain the kind of anesthesia required, surgical facility, and costs.

Your surgeon will begin by asking for a complete medical history, and then examine the skin and fat around your eyes. He/she may also examine your eyesight, peripheral vision, and tear ducts. You should tell him/her about any related symptoms you may have, including any dryness of the eyes, changes in vision or eye pain.

Surgical Procedures
The upper eyelid incision is made in the natural skin fold. Excess skin and fatty tissue is removed, the muscles and orbital septum (a thin connective tissue membrane) may be tightened, and the incisions are carefully closed. The external incisions are hidden within the natural fold of the upper eyelids. Much of the operation is done with a very fine electrocautery, which controls any bleeding. The skin is then closed with very fine sutures.

Post –Operative Care
Your eyes will likely be lubricated and covered with bandages after surgery. Oral medications will help with pain and discomfort. Report any unmanageable pain to your doctor immediately. The doctor will also prescribe an antibiotic to prevent infection.
You will receive a list of instructions to follow for several days, including:
·        the proper way to clean your eyes
·        use of cold compresses
·        use of lubricating eye drops
·        head elevation
·        no makeup
·        no alcohol
·        no television
·        no contact lenses
Stitches are removed about a week after surgery. Bruising and swelling will continue to decrease after stitches are removed. Sunglasses and protective sun block are required for several weeks.
Normal activities can resume after about 10 days, though strenuous activities such as lifting, bending and exercise can resume after about three weeks.

Possible Risks and Complication
Significant complications from upper eyelid blepharoplasty are infrequent. As witany surgical procedure, however, there is always a possibility of infection, or reaction to the anesthesia. Retrobulbar hematoma - (bleeding behind the eye) - rare, but can cause loss of vision.Temporary problems with excessive tearing .Decreased sensation in the eyelid. Dryness, burning, stinging, gritty sensation in your eye(s) .Prominence or firmness of the scars. Asymmetry in healing or scarring .Milia or whiteheads where the sutures emanate from the skin. Difficulty closing their eyes completely; in rare cases, this condition may be permanent. Further surgery may correct this problem.You can help minimize certain risks by following the advice and instructions of your plastic surgeon, both before and after your eyelid surgery.

  Stop smoking and consuming alcohol for at least a week before surgery. Talk to your surgeon about ways to quit smoking for good.
Your plastic surgeon will likely suggest that you stop taking certain medications, including aspirin, non-steroidal anti-inflammatory drugs (NSAIDs) and blood thinners, all of which can increase bleeding risk.
Certain herbal remedies may also increase bleeding risk. Just because a product or preparation is "all-natural" does not mean it is safe. For example, vitamin E, omega-3 fatty acids, green tea and gingko biloba all may increase bleeding risk during and after surgery.
Make sure you tell your surgeon about everything that you are taking. Never stop taking any medication abruptly without first speaking to your doctor.
